In-Home Care Services

Care Management
Our professional staff provides service coordination care through an assessment plan of your needs. Care Management is a service that assists older adults in gaining access to needed services. The care manager assesses needs for services and then coordinates them in a timely manner. These services may be offered through Area Agency on Aging (AAA) programs and/or other community resources. The goal of this process is to preserve self-support and, as much as possible, increase self-sufficiency.

 

Family Care Giver Support  
Families are the major provider of long-term care, but research has shown that caregiving exacts a heavy emotional, physical and financial toll. Experience, Inc. provides programs and services that can help by providing respite care and financial reimbursement for those who qualify. The Caregiver Support Program (CSP) has two distinct components. The first is especially designed to reinforce caregivers who are generally capable of managing the care for their relative who is over the age of 60. To be eligible for the program, an individual, 18 years or older, must be caring for a physically and/or mentally impaired relative. The second component offers support to individuals for their grandchildren. For both components, the program is centered around offering financial reimbursement for expenses the caregiver pays out for services and supplies associated with the care of the older adult/grandchild. Expense reimbursement for this program is based on a sliding scale of household income.

Experience, Inc. provides programs and services that can help by providing respite care and financial reimbursement for those who qualify.

 

Home Delivered Meals
Meals are delivered Monday – Friday to older adults, those with disabilities, or anyone who qualifies by the inability to prepare a nutritious meal for themselves. Donations are encouraged and gratefully accepted for this service. In Home Meal Service provides meals to individuals in their home. Each meal is well balanced, nutritious, and contains at least one-third of the current recommended daily allowances as established by the Nation Academy of Sciences.  The In Home Meal Service will help ensure minimum levels of sound nutrition for consumers who have difficulty providing their own nutritional needs.  They are available in order to avoid malnutrition, maintain self-sufficiency and delay or avoid entry to a long-term care facility. Older adults age 60 and over are eligible.

Personal Care and Home Support
Personal Care Services provides “hands-on” care in the home to individuals 60+ who are unable to independently meet their personal hygiene needs or perform activities of daily living. The Home Support Program provides light housekeeping, laundry, and shopping assistance to frail, older adults living in their own homes.

OPTIONS is a program that allows individuals to receive services and supports in their homes. The OPTIONS Program serves individuals who are either financially or clinically ineligible for Medical Assistance (MA) Long-Term Services and Supports. OPTIONS services are provided to eligible consumers aged 60+ to assist them in maintaining independence at the highest level of functioning in the community and help delay the need for more costly care/services.

Program Eligibility requirements, an individual shall be:

Age 60 and older – experiencing a need for personal care assistance due to frailty in regard to physical and/or mental status that impacts daily functioning – A Pennsylvania resident – A U. S. citizen or lawful permanent resident. This is a cost sharing program where the consumer may have to help with the cost of care, depending upon income. A sliding fee scale based upon the person’s (and their spouse’s, if applicable) income is used to determine what percentage, if any, they may have to pay.

 

Personal Emergency Response System
The Personal Emergency Response System or PERS, are home devices that connect older adults to a 24-hour call center with the push of a button. The transmitter is typically worn on a neck pendant or wristband, and it sends a signal to a receiver that’s connected to the home telephone line. One push of the button alerts the staff at the call center.
