Care Management

Care Management is a service that assists older adults in gaining access to needed services. The care manager assesses needs for services and then coordinates them in a timely manner. These services may be offered through Area Agency on Aging (AAA) programs and/or other community resources. The goal of this process is to preserve self-support and, as much as possible, increase self-sufficiency.
