In-home Care Services
Care Management
Our professional staff provides service coordination care through an assessment plan of your needs. The Care Manager assesses needs for services and then coordinates them in a timely and appropriate manner. These services may be offered through the Area Agency on Aging (AAA) programs and/or other community resources.
Family Care Giver Support
Families are the major provider of long-term care, but research has shown that caregiving exacts a heavy emotional, physical and financial toll. Experience, Inc. provides programs and services that can help by providing respite care and financial reimbursement for those who qualify.
Home Delivered Meals
Meals are delivered Monday – Friday to older adults, those with disabilities, or anyone who qualifies by the inability to prepare a nutritious meal for themselves. Donations are encouraged and gratefully accepted for this service.
Personal Care and Home Support
Personal Care Services provides “hands-on” care in the home to individuals 60+ who are unable to independently meet their personal hygiene needs or perform activities of daily living. The Home Support Program provides light housekeeping, laundry, and shopping assistance to frail, older adults living in their own homes.
Personal Emergency Response System
The Personal Emergency Response System or PERS, are home devices that connect older adults to a 24-hour call center with the push of a button. The transmitter is typically worn on a neck pendant or wristband, and it sends a signal to a receiver that’s connected to the home telephone line. One push of the button alerts the staff at the call center.
The Intake Specialist can provide you with information on support services available to allow one to remain safely at home. Intakes for all the programs can be done over the telephone or through a scheduled appointment in the office.
In order to be referred to the proper program, you will be asked, among other necessary information, to provide information on your income and assets. If you are homebound, an Outreach Visit can be done.
Information packets can be mailed to you with an explanation of the programs, as well as information on the financial criteria that needs to be met to qualify for services.
If you do not meet the criteria, the Intake Specialist has a wealth of information on other resources available in the community. If someone needs to be placed in a facility from their home, the Intake Specialist is the person to contact so the necessary steps can be taken.
