In-Home Care Services
Care Management
Our professional staff provides service coordination care through an assessment plan of your needs.
Family Care Giver Support
Families are the major provider of long-term care, but research has shown that caregiving exacts a heavy emotional, physical and financial toll. Experience, Inc. provides programs and services that can help by providing respite care and financial reimbursement for those who qualify.
Home Delivered Meals
Meals are delivered Monday – Friday to older adults, those with disabilities, or anyone who qualifies by the inability to prepare a nutritious meal for themselves. Donations are encouraged and gratefully accepted for this service.
Personal Care and Home Support
Personal Care Services provides “hands-on” care in the home to individuals 60+ who are unable to independently meet their personal hygiene needs or perform activities of daily living. The Home Support Program provides light housekeeping, laundry, and shopping assistance to frail, older adults living in their own homes.
Personal Emergency Response System
The Personal Emergency Response System or PERS, are home devices that connect older adults to a 24-hour call center with the push of a button. The transmitter is typically worn on a neck pendant or wristband, and it sends a signal to a receiver that’s connected to the home telephone line. One push of the button alerts the staff at the call center.
Supportive Services
PA Medi
PA Medi is the State Health Insurance Assistance Program (SHIP) for Pennsylvania’s Medicare Beneficiaries. PA Medi will help Pennsylvanians with Medicare understand their health insurance options and help them make sound decisions about what is best for them. Aside from Medicare/Medicaid eligibility, the PA Medi program will screen for eligibility for all other programs.
Outreach
The outreach goal is to provide information about the availability of resources and services, as well as linking the person to the appropriate agency through the use of a home visit.
Ombudsman
Pennsylvania Ombudsmen are federally mandated, legally-based, and state certified to actively advocate and give voice to people who receive long-term care services, whether delivered in the community or a facility-based setting.
Protective Services
These services are provided to individuals age 60 and older who are at risk of physical, psychological, and sexual abuse, neglected or self-neglect, and/or exploitation. Individuals may reside in the community or within a licensed facility. The program includes a broad range of legal, medical, and social intervention on behalf of the older adult. The goal of protective services is to reduce or eliminate the risk of abuse.
